    State Pin Pre-order Form
 
Creativity in Arkansas is sponsoring the sale of pins for the 2011 state tournament.   Although pins will be sold after March 18th, and at the tournament on April 9th, they will be on a first come first serve basis.   The cost of each pin will be $3.00.   You can ensure that you will have plenty of pins for your team members, team members' family, etc. by pre-ordering via e-mail your master order form on or before March 18th, to sales coordinator, wpeters@sdale.org.
 
COACHES:   You are responsible for coordinating your team's pin order details with team members, parents, etc.   We request that you send only one master order form to the e-mail address below that outlines your team's pin needs.   We would like the money to be collected for each team order, then send one check or money order for full payment to be received on or before March 25th.   No cash please. This will help us ensure accuracy in order fulfillment.   If you have any questions, please contact your state director.   Thanks for your cooperation!
