Goal/s: Establish confidence upon interaction, promote patient’s self-esteem, provide a high level of trust, promote self-enhancement through life confession experience, and gain working trust and relationship Date: 1/30/2013

Time: 15-20 minutes
Venue: National Center for Mental Health, Pavilion 4 (Forensic Ward) Patient’s General Appearance: The patient seems to have a sad mood as seen by his affect and the way of his speaking, which is not as lively as our first meeting. He still follows with the program and shares answers but his overall change is noticeable. The patient was much more reserved and took long pauses to answer the questions resulting in a short interview. Nurse| Patient| Therapeutic Communication| Analysis|

N: Magandang umaga Addong! (waves to px)N: Kamusta ka naman po?N: Ano po dahilan ng lungkot na inyong nararamdaman ngayon?N: Sino po ba ang bumibisita sa inyo dati?N: Ah, ganoon po ba? Ano naman po mga ginagawa niyo para malibang kayo dito?N: Bale, naaalala niyo po ba ang mga napag-usapan natin ngayon? Tungkol po sa mga nararamdaman niyo po ngayon na malungkot po kayo dahil walang bumibisita pero nalilibang mo naman ang sarili mo sa mga gawaing pang-araw araw.| P: Good morning din po sir!
