THE PROBLEM AND IT’S BACKGROUND
Today we are living in a Technology-based world. But still, most of the dental clinics make use of paper-based medical records to put all information of the patient. Medical record is the case history of a medical patient as recalled by the patient. Another definition is a record of a patient’s medical information (as medical history, care or treatments received, test results, diagnoses and medications taken). In this country, almost all of dental clinic uses a paper-based recording system, and because of this, the records that the clinics have sometimes jumbled up, missing, or ended up having errors. Also, having a paper-based system in a large clinics or hospitals that have a common problem, that is when the records of the patients piled up, a particular file of a patient is hard to find. Also, it is hard to edit and keep it on track of the patient’s health.
But with the use of computers, we can build a system that is suitable for the local, which is the dental clinic, to manage the record of the patients, like the systems that are used by advanced countries like America and Japan.
A record keeping system is a systematic procedure by which the records of an organization are created, captured, maintained, and disposed of. This system also ensures their preservation for evidential purposes, accurate and efficient updating, timely availability, and control of access to the them only by authorized personnel.
There are three different formats of record keeping systems: paper, micrographic and electronic. The record keeping-systems are composed of the following: people, policies, procedures, tools, technology, ongoing supporting education, and maintenance. With these facts in mind, the researchers come up to a proposal which is entitled “Tarlac State University Digital Dental Registration and Record Keeping System” for the locale which is the Tarlac State University Dental Clinic.
