How to ask a professor to be considered a candidate for a PhD degree

My name is XXXXXXX I’m a law undergraduate student from University of San Marcos in Lima, Peru. I graduate with a 4.0 GPA in my class. I worked in XXXXXX, (in this section you want to add you extracurricular activities, such you have donate time to a nursing home, etc, etc ). I am currently developing my undergraduate thesis that is the last step to graduate and be certified as a lawyer. My thesis is about the supervening contractual imbalance in the Peruvian legal system, which has some modifications in the Italian and German systems, which this last one is explained with the theory of Geschäftsgrundlage.
