Our ZorgConsulent is also ready and willing to offer you additional information and advice on health, illness, prevention and health care. Terms and conditions This booklet lists all policy terms and conditions of the ONVZ Vrije Keuze Zorgplan 2012. What are you entitled to? What services does ONVZ provide? How can you take advantage of these services? The terms and conditions will apply as of 1 January 2012 and will be valid until any changes take effect. Your policy documents list the terms and conditions that apply to you. You can also find your details at www.MijnONVZ.nl. Summary The ONVZ Vrije Keuze Zorgplan is made up of the ONVZ Vrije Keuze Basisverzekering and a range of different supplementary plans. This booklet lists the levels of coverage (part B) and the ONVZ health-care services (part C) for each health-care plan. The general provisions are listed at the front (part A). The general provisions set out the agreements with regard to your rights (e.g. privacy) and obligations (e.g. premium payment, notification of change of address). Part A-1 applies to all health-care plans, with certain terms being excluded from the supplementary plans. Part A-2 applies to the supplementary health-care plans only.
