History.—Family Health Care Clinic was funded in 1977 as a non-profit entity. The community based organization has 501 C-3 IRS tax exempt status. Family Health Care Clinic is governed by a Board of Directors whose membership is made up of 51% patients. The board hires a Chief Executive Officer who manages the day-to-day operations. include Family Practice and Dental. In 1978, services were expanded to

In 1989, OB/GYN services were added. Through an

expansion grant in 1991, the first satellite was established in New Hebron. In 1992, three (3) satellites were opened with generated revenue. In 1994, the Family Health Care Clinic was

awarded an expansion grant to open a clinic in Monticello. Dental services were expanded to Monticello in 2003. The Smith County site was funded in September 2007 and opened

December 3, 2007. In 2008 we changed our scope and added clinics in Grenada, Winona, and Calhoun City. We are currently located in all Congressional districts in Mississippi. Currently there are a total of seventeen (17) sites located in fourteen (14) counties. Our Alabama clinics opened in 2009 and consist of six (6) clinics located in five (5) counties of Northwest Alabama. We are currently located in two (2) congressional districts in Alabama. They are District four (4) and District five (5). EMR History: Our clinic has been operational with a first generation EMR (implemented 2002) for over seven (7) years and currently offer a second generation (implemented September 2009) CCHIT certified EMR to 23 medical, behavioral, and dental sites. As well as 16 Head Start sites.
