Prior to the current global economic downturn,third world countries have been grappling with the problems of underdevelopment,mismanagement corruption and recurrent economic crises. The impact of this is no more felt then in the health care industry where I work. As a medical doctor who has had the opportunity to practice but in the big city and in the rural areas of a third world country like Nigeria,two major things strike me; first in the area of funding for public health program, which has drastically reduced as a result of reduction in funds from donor agencies and the inability or unwillingness of the government to make up the shortfall.Programs such as HIV/AIDS prevention, and control,family planning initiatives and even programs to combat childhood killer diseases have all suffered or are on the verge of suffering from an acute shortfall of funds,this has had a direct on health care delivery in Nigeria. Another effect of the economic downturn is the fluctuating price of oil on the global market.Nigeria is a mono product economy,with almost 90percent of our exports got from oil. Any fluctuations on the oil market has ripple effects on the economies of our country and funding for public health institutions like the the ones I work in. I currently work in a tertiary health institution located in a rural part of Nigeria,poor government funding and the challenge of attracting skilled personnel to a rural community where there is a dearth of social infrastructure means that the hospital has to devise creative ways of making its offer lucrative to skilled hands in the face of dwindling public funds. One of the ways of doing this is to ensure medical personnel can engage in online discussions of interesting medical/surgical cases with contemporaries in other parts of the country and indeed other parts of the world.
