Lark St., Boston, Massachusetts 12345, and social security number is 951-53-8462. 14 Now on motion of Patty Bean, Plaintiff, it is: 15 ORDERED AND ADJUDGED that the Referee’s Report, if any, is hereby confirmed; and it is further 16 ORDERED, ADJUDGED AND DECREED that the application of plaintiff is hereby granted to dissolve the marriage between Patty Bean, plaintiff, and David Bean, defendant, by reason of: 17 the relationship between Plaintiff and Defendant has broken down irretrievably for a period of at least six months pursuant to D.R.L. 170(7); and it is ORDERED AND ADJUDGED that the Plaintiff shall have custody of the minor children of the marriage, i.e.: 18 Name Date of Birth David Bean, Jr. June 5. 2005 Patricia Bean July 1, 2008 and it is further; 19 ORDERED AND ADJUDGED that the Defendant shall have visitation with the minor children of the marriage according to the following schedule: Weekend visitation every other weekend.
