A DA Form 1687 (Notice of Delegation of Authority-Receipt for Supplies) is the Army form used to delegate authority to an individual in order to receive supplies on the Commanders behalf in support of missions. A current copy of the Unit Assumption of Command memorandum must also accompany the DA form 1687 document. The most current date for the DA 1687 is May of 2009. The importance of the DA form 1687 is to delegate authority to individuals of the commanders’ discretion to act in his behalf, there by freeing the Commander to accomplish other tasks. The DA from gives representatives of the commander the right to request and / or receive materiel. The option of requesting or receiving materiel can individually be authorized or unauthorized to each representative according to the Commanders discretion. All blocks must be filled out on the DA form 1687. The date block in the upper right hand corner of the document must have the current date. The organization receiving supplies block will have the organizations name. The location block will state the post that the unit is currently stationed at, The name block will have the names of the representatives in the manner described. The authority REQ / REC blocks will state YES or NO in each block. The next set of blocks will contain the signatures and initials of the corresponding representatives. The authorization by responsible supply officer or accountable officer block will have the appropriate blocks checked and the correct statement place after “THE AUTHORITY TO”. The units UIC will be placed in the next block. The unit DODAAC and account number in the following block. The unit Commanders name as stated will be place in the bottom left block. The next two blocks will consist of the Commanders Grade and contact number. The expiration block will have the day prior to the following year of the date indicated in the top right corner.
