Identifying Information

Sarah is 19 years old white female who is referred by her physician. Sarah is 5 foot 9 inches tall with an ideal body weight of 120 pounds. Sarah is single, and she is not dating anyone right now. She graduated from high school over a year ago, and currently she works at MFD Model & Talent Agency as a fashion model. Based on information gathered from her interviews, Sarah has a three-year history of sever anorexia nervosa. Her physician has evaluated her earlier and reported her medical condition as irregular heartbeat, low blood pressure, and potassium deficiency. Presenting Concerns

Sarah agreed to come to therapy because she believes she made herself physically weak, and she is worried about losing her modeling career. She was told to leave work on a few occasions because of her sickly appearance. Sarah started dieting and became anorexic when she was 16-years old, and began losing weight quickly. Her weight had dropped in a few months from 160 to 120 lbs. Since then she has been experiencing hair loss, dizziness, low energy level, low blood pressure, irregular heartbeat, excessive headaches and amenorrhea. Sarah reported that she does not feel very thin, and she denies the seriousness of her weight loss. Further, during the therapy she constantly complained about being overweight, being tired all the time, and she is extremely worried about losing “chunks” of hair everyday. Sarah had previously been treated unsuccessfully in two eating disorder clinics. She explained that previously she never wanted to get better; she “always wanted to be so skinny that people would stop on the street and stare at her.” During our first therapy session Sarah seemed somewhat motivated to change her eating habits, and wanted to start a new treatment plan.
