Introduction
All the information that has been obtained to complete this report has been gained by way of parental consent due to the age of the patient. The opportunity to withdrawal from the report was also given to the parent. In order to protect the privacy of the patient and their whanau, and to maintain confidentiality, pseudonyms have been put in place. This will enable the reader to gain a clear perspective of each individual.
    Therefore all subjective data has been carefully collected and recorded in the presence of a Registered Nurse and parental guidance as well as personal communication from the patient.
    It was my first day of practicum in a community clinic and I was very fortunate to meet a young one year old boy I will refer to as Tama. Tama was brought in to clinic first thing morning by his mum. The Registered Nurse Maia, whom I was assigned to suggested Tama would be a good case to follow up as there were obvious signs for further contact or recall to the clinic.
    So therefore before I was able to enter the room, Maia approached Tama’s mum to see if it would be alright for me, a second year student nurse to observe Tama’s condition. After consent was gained Maia and I sat with mum to establish a background view of Tama leading up to their visit to the clinic.
    Mum recalls it all started on the previous Monday when Tama had a bit of a cough and began to get worse around Thursday when she took him in to the family doctor. Her doctor prescribed some medication namely phenergan and pamol which did seem to work for a few days. Eventually Tama’s coughing became worse and he stopped eating and was barely drinking as well as looking really drained and tired. So mum decided to take Tama to the outpatient department on the Sunday where they were advised he had a cold and to continue administering his current medication and sent them home.
